Applications must be typed or printed legibly, preferably in black ink.

Applications must be complete. Incomplete applications will not be processed. All statements and information provided in the application must be correct and true to the best of your knowledge and may be subject to investigation by the WPCSOCC or other appropriate authorities.
Applications must be postmarked at least 30 days prior to the date of the exam.
Applications must be signed by both the applicant and the applicant's supervisor. All attachments that address Section V of this application must be signed by both the applicant and the applicant's supervisor.
Applications that do not contain all required signatures will not be processed.
Where eligibility to take an examination is based on a college degree, a copy of an official transcript for all course work leading to the conferring of that degree must be included with the application.
