Unit :: NPDES Phase II Stormwater Program
Instructions for logging on to the on-line Phase II Annual Reporting form
1) Once at the site, click on Stormwater Assessment Login.
2) The "login" is your permit number and the default password is "password.” You’ll be prompted to
enter a new password.
3) Once you've logged in, be sure to select the “save/edit report in review mode.” This mode allows you to enter and edit entries, skip questions, and come back later to finish the report without submitting it to DWQ. The data that you enter while in the review mode will be saved automatically until you are ready to submit your report to DWQ.
4) You can only submit your report to DWQ from the "save/edit report” mode.
