APPLICATION FORMS - INDIVIDUAL PERMITS

All new and expanding facilities MUST submit an Engineering Alternatives Analysis (EAA) in addition to any required applications.

Municipal POTW and 100% Domestic Facilities

 Major industrial facilities with stormwater outfalls will also need to fill out EPA Form 2F. A copy of this form should be submitted for every stormwater-only outfall covered under the NPDES permit.

 All other minor industrials : check NC Form C-MI
